As a CalAIM provider, we specialize in Enhanced Care Management (ECM), Community Support Services, and Community Health Worker (CHW) programs. Our mission is to bridge the gap in healthcare access by delivering comprehensive, person-centered services that empower individuals to lead healthier lives.
With established partnerships with Molina Healthcare and Inland Empire Health Plan, we work collaboratively to provide holistic, quality care that meets the unique needs of our clients.
We serve communities in Los Angeles, San Bernadino and Riverside.
